{"id":5109,"date":"2021-08-30T10:23:00","date_gmt":"2021-08-30T10:23:00","guid":{"rendered":"https:\/\/cdafound.org\/?p=5109"},"modified":"2025-01-14T21:38:56","modified_gmt":"2025-01-14T21:38:56","slug":"epidemiology-natural-history-and-outcomes-of-primary-sclerosing-cholangitis-a-systematic-review-of-population-based-studies","status":"publish","type":"post","link":"https:\/\/cdafound.org\/fr\/epidemiology-natural-history-and-outcomes-of-primary-sclerosing-cholangitis-a-systematic-review-of-population-based-studies\/","title":{"rendered":"\u00c9pid\u00e9miologie, histoire naturelle et r\u00e9sultats de la cholangite scl\u00e9rosante primitive\u00a0: examen syst\u00e9matique des \u00e9tudes bas\u00e9es sur la population"},"content":{"rendered":"<h2 class=\"wp-block-heading\">R\u00e9sum\u00e9<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Contexte et objectifs&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Le but de cette \u00e9tude \u00e9tait de quantifier l&#039;\u00e9pid\u00e9miologie mondiale de la cholangite scl\u00e9rosante primitive (CSP), ainsi que l&#039;incidence de la transplantation h\u00e9patique, du cancer et de la mort, gr\u00e2ce \u00e0 un examen syst\u00e9matique solide des donn\u00e9es bas\u00e9es sur la population.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">M\u00e9thodes&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Nous avons effectu\u00e9 des recherches dans MEDLINE et EMBASE jusqu&#039;au 30 juin 2020 inclus pour identifier les \u00e9tudes bas\u00e9es sur la population rapportant l&#039;incidence et\/ou la pr\u00e9valence de la CSP. Les \u00e9tudes qui ne rapportaient pas de donn\u00e9es originales, ou portant exclusivement sur la maladie d&#039;apparition p\u00e9diatrique (\u00e2ge de diagnostic &lt;\u00a016\u00a0ans) ou exclusivement sur la CSP associ\u00e9e \u00e0 une maladie inflammatoire de l&#039;intestin ont \u00e9t\u00e9 exclues.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">R\u00e9sultats&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Sur 4922 \u00e9tudes publi\u00e9es, 17 remplissaient les crit\u00e8res d&#039;inclusion\u00a0; 16 documentant l&#039;incidence et 14 la pr\u00e9valence. L&#039;incidence la plus \u00e9lev\u00e9e de CSP signal\u00e9e a \u00e9t\u00e9 signal\u00e9e en Europe du Nord (Finlande, 1,58 et Norv\u00e8ge, 1,3 pour 100 000 habitants, respectivement) et en Am\u00e9rique du Nord (Minnesota, 1,47); le plus bas \u00e9tant observ\u00e9 dans tout le bassin m\u00e9diterran\u00e9en (Italie, 0,1). La pr\u00e9valence variait de 31,7 en Finlande et 23,99 au Minnesota, \u00e0 1,33 \u00e0 Singapour et 0,0 en Alaska. Parmi les \u00e9tudes faisant \u00e9tat d&#039;une occurrence temporelle, une augmentation de l&#039;incidence de la maladie a \u00e9t\u00e9 observ\u00e9e en Am\u00e9rique du Nord et en Europe du Nord (4 \u00e9tudes), parall\u00e8lement \u00e0 une augmentation de la pr\u00e9valence au fil du temps (4 \u00e9tudes). L&#039;incidence et les risques pour les r\u00e9sultats cliniques ont \u00e9t\u00e9 pr\u00e9sent\u00e9s par 9 des \u00e9tudes incluses. La survie m\u00e9diane sans greffe variait de 9,7 (\u00c9tats-Unis) \u00e0 20,6 ans (Pays-Bas), avec des taux de mortalit\u00e9 standardis\u00e9s de 2,5 et 4,2 par rapport \u00e0 la population t\u00e9moin. L&#039;incidence standardis\u00e9e du cholangiocarcinome variait de 235 (Finlande) \u00e0 398 (Pays-Bas).<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Conclusions&nbsp;<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Les estimations de l&#039;incidence et de la pr\u00e9valence de la CSP varient, la plupart des \u00e9tudes \u00e9tant men\u00e9es en Am\u00e9rique du Nord et en Europe occidentale\u00a0; ce dernier montrant une augmentation constante de l&#039;apparition de la maladie au fil du temps.
